If you have insurance, please be familiar with your benefits. You will need to provide this information during your pre-registration call. In addition, prior to treatment, please inform us if your insurance requires pre-certification, pre-authorization or a second opinion; or your treatment may be cancelled or delayed.
As a courtesy, we will submit all insurance claims for you; provided that we have all of your information. Please remember, our relationship is with you, not your insurance company. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account. Payment not covered by your insurance must be paid at the time of treatment, unless you have made arrangements with our office.
